How do I go about registering?

• If you have questions after reviewing the information posted on the website, please contact Maria maria@ima-missions.org  for any additional information needed.
• Please make sure the project you are interested in is sponsored by IMA.
•  If it is sponsored by GHO you should follow the link to GHO (Global Health Outreach).
• Fill out an application, either online, or download/print and send it in to IMA.
• Await acknowledgement of receipt and acceptance of your application from Maria.
• If you don’t get an acknowledgement from Maria within two weeks, please contact IMA by phone (877) 251-0526 to make sure that your application has been received.
• Please don’t buy airline tickets before you have confirmation from IMA that you will be part of the project.
• There are specific guidelines for each team on airport arrival/departure times, as typically there is a two-hour bus trip involved to/from final destination --please get these from Maria.
• Medical participants need to have their credentials on file at the IMA office. 
• If participating for the first time, please make sure you send in a copy of your diplomas, CV, and current state license.  Surgeons must provide copy of privileges.
• If repeating participation and credentials are on file, only current copies of state licenses and privileges are needed.
• Signed Release of Claims and Code of Conduct need to be sent in.
• Copy of flight itinerary needs to be sent in.
• A deposit donation of $100.00 needs to be sent in at time of registration.  This can be done through PayPal link on the website using a credit card, or by mailing a check.  Full amount may be sent in at one time, once your acceptance/participation has been confirmed.
• Balance of donation needed to cover land portion of the mission project --$700.00 for one week, $900.00 for two weeks—needs to be in IMA office by one month before starting date of the project.

Checklist –these items need to be in the IMA office by one month before departure:

• Application
• Signed Release and Code of Conduct
• Copies of Credentials and State License if in the medical field.  Credentials need only be submitted once.  Copy of state license has to be current.
• Copy of Flight Itinerary
• Total Donation of $800.00 for one-week or $1,000.00 for two-week participation.
